For 15 years Community Together CIC has been helping people to ‘Grow Well’, ‘Live Well’ and ‘Age Well’
They are continuing on their journey in supporting communities as they work closer with their NHS Partners for integrated care and person centre plans working together for better health outcomes, helping people to live as independently as possible for as long as possible in connected communities.
